Bra-line back lift: the complete patient guide

The bra-line back lift — also called an upper back lift — is the operation designed for a problem that has no non-surgical answer: rolls of loose skin across the upper and mid back that survive weight loss, defy exercise, and show through every fitted top. The operation in five lines

Why back rolls happen — and why the gym can't fix them

The skin of the back is anchored at the spine and along the sides by fibrous attachments; between those anchors it drapes over the ribcage and the muscles beneath. Weight gain stretches it; weight loss, pregnancy and time leave it longer than the frame it covers. The result folds — typically at bra-band level, where the band itself compresses tissue into a roll above and below. The fold is skin, not muscle: no exercise shortens skin, and weight loss, past a point, makes the fold looser rather than smaller.

There are two different rolls that look alike in clothing. A fatty roll under elastic skin — common in younger patients — flattens once the fat is removed by liposuction. A loose-skin roll — after significant weight loss, after pregnancies, or with age — is a fold of redundant skin that liposuction would only deflate. The pinch test at consultation tells them apart, and it decides everything that follows.

The operation, step by step

Under general anaesthesia, the planned ellipse — drawn with you standing, in your own bra, the day before — is excised across the back at bra-band level. The tissue above and below is mobilised so the closure spreads tension across the whole back rather than concentrating it at one line. Where the roll continues toward the sides, the excision follows it, curving with the natural fold under the arm. Liposuction of the surrounding back, flanks and the roll beneath the bra line blends the transition into a smooth contour. Closure is in layers with dissolving deep sutures that carry the tension, so the skin sutures carry none. Drains are used selectively and removed within days. The operation takes 2–3 hours alone; longer when combined with arm or breast surgery as part of an upper body lift.

Two facts that decide your result: (1) the scar quality is set by tension management and placement — the planning, not the stitching; (2) the contour quality is set by combining excision with liposuction — removing the fold alone leaves a tightened strip, not a smooth back.

The scar, honestly

A horizontal line across the back at bra-band level, hidden under the band of a bra or bikini top, extending as far toward the sides as your excess requires. It is red and raised for months, fades through the first year and reaches its final pale line at 12–18 months. It never disappears. Who it's for — and who should not have it

Candidates are patients whose rolls are made of loose, foldable skin; at a stable weight for 6+ months; non-smoking for at least 4 weeks either side of surgery; with realistic expectations of a smooth back carrying a hidden scar. Yes — the terms describe the same operation: excision of the upper/mid back roll through a horizontal incision at bra-band level. 'Bra-line' refers to where the scar is placed.

How long do the results last?

Permanently, in the sense that removed skin does not return. Ageing and weight change continue; stable weight and sun protection keep the result and the scar at their best for decades.

Will I need drains?
