Recovery: week by week

Recovery from a bra-line back lift is shorter and gentler than most patients expect. One rule governs it: protect the closure from stretch. Almost every instruction below — arm height, lifting, sleeping position — is that rule applied to a moment of the day.

Recovery in five lines

Day 0 — surgery day

Surgery takes 2–3 hours under general anaesthesia — longer with liposuction, arm or breast procedures. You wake in the compression vest with dressings along the closure and, when used, small drains. Walking starts the same evening; tightness across the back is the dominant sensation and is controlled with scheduled oral analgesics. One hospital night with monitoring and clot prevention.

Days 1–5 — hotel recovery

Back to your suite with daily checks at the clinic: wound review, garment adjustment, drain output. Short walks several times a day; upright posture is allowed (unlike a tummy tuck) — it's the arms that are restricted: below shoulder height, no reaching behind or overhead, no pulling doors or lifting bags. Showering is usually cleared at 48 hours with waterproof dressings. Sleeping: on your back with pillow support under the knees, or on your side — both are designed for; not on your front.

The three-week arm rule: every time you raise your arms above your shoulders you stretch the closure across the back. Lifting, reaching a high shelf, pulling a suitcase, drying your hair overhead — all wait until week 3. Wardrobe planning (front-opening tops) is the practical answer.

Days 6–8 — fly home

Final check, drain removal if not already done, and fit-to-fly clearance. Compression stockings and hourly aisle walks on the flight; a wheeled case, airport assistance, and nothing lifted into an overhead locker. The flying article covers the details.

Weeks 2–3 — desk work and scar care

Sedentary work resumes at 7–10 days, earlier for many. Sutures are dissolving; once the incision is sealed (usually week 2–3), silicone gel or tape starts and continues for months. Arms still below shoulder height; driving waits until you can turn and reach without pulling — typically week 3.

Weeks 4–6 — range of motion

Arm elevation returns progressively; gentle stretching is encouraged as tolerated. Light housework, longer walks, normal sleeping. The new contour is visible as swelling settles. The vest comes off at week 6 in most plans.

Week 6 onward — exercise and refinement

Cardio and lower-body strength return at week 6; upper-body and back training last and gradually; swimming once the scar is fully sealed. Residual swelling and firmness resolve over months 3–6; numbness around the scar recovers over the first year. The scar fades through months 12–18 to its final pale line — the scar guide maps it.

Normal versus 'message now'

Normal: tightness, bruising into the flanks, mild swelling that is worse in the evening, numbness around the scar, itching as nerves recover. Message the same day: a soft swelling that increases (possible seroma), spreading redness or heat, fever, a wound edge that opens, calf pain or breathlessness. Almost everything in the first list is the recovery working; everything in the second is checked, not watched.

Questions patients ask

How painful is a bra-line back lift?

Tightness rather than sharp pain; most patients manage on paracetamol and an anti-inflammatory after the first day or two and are off stronger medication within days.

When can I sleep on my back?

From the first night — the closure tolerates lying on your back with pillow support, and side-lying is fine. Front-lying waits until the scar is sealed and comfortable, usually week 3–4.

When can I wear a normal bra again?

The compression vest replaces a bra for 4–6 weeks. After that, a soft bra whose band sits on or just below the scar is fine; underwires that press directly on the line wait until the scar is soft, around month 2–3.
