Back lift surgery explained: types, terms, and which one you need
'Back lift' is used online for four different operations. Patients arrive asking for one and needing another. This page defines each in plain language, says which part of the back it treats, and maps the roll you have to the operation that removes it.
- Bra-line (upper) back lift: removes the roll at bra-band level through a horizontal scar hidden under the band — the subject of this site.
- Lower back lift: removes rolls above the buttocks/flanks through a scar at the beltline; usually the back half of a lower body lift or extended tummy tuck.
- Lower body lift (belt lipectomy): a circumferential lift — tummy tuck in front, lower back and buttock lift behind — for massive-weight-loss skin around the whole waist.
- Back liposuction: removes fat without removing skin; the right answer only when skin is elastic.
- Upper body lift: bra-line lift + arm lift and/or breast lift as one plan after massive weight loss.
Upper back — the bra-line roll
The fold at bra-band level, running toward the sides under the arm. It's the most searched 'back roll' and the one this site is about: excised through a scar planned under the bra band, usually with liposuction of the surrounding back. The complete guide covers the operation.
Lower back — the beltline rolls
Rolls that sit above the buttocks and wrap the flanks are a different tissue problem, tethered to the pelvis rather than the ribcage. They are treated through a beltline incision — either as the back half of a lower body lift (with a tummy tuck in front) or as an extended tummy tuck whose scar continues around the flanks. The lower back lift page explains who needs which.
Both — the massive-weight-loss back
After 40+ kg of loss, upper and lower rolls coexist. They are treated in stages: typically the lower body lift first (the larger operation, and the one that changes the waist), then the bra-line lift with arms and breasts as an upper body lift 3–6 months later. Staging is a safety decision; the order is discussed at assessment.
Neither — the liposuction back
Firm, fatty rolls under elastic skin — common under 40 with a stable, moderate weight — flatten with liposuction alone and need no scar. The lift-versus-liposuction guide gives the pinch test that separates skin from fat.
Mapping your roll: above the bra band → bra-line back lift; at the beltline → lower back lift / lower body lift; both → staged; firm and elastic → liposuction. Photos from behind with arms relaxed and raised settle it at assessment.
Terminology patients meet online
Upper back lift and bra-line lift are the same operation. Bra roll surgery, back fat surgery and back roll removal are lay terms that can mean any of the four. Belt lipectomy and circumferential body lift mean lower body lift. Torsoplasty is an older umbrella term. The glossary defines each.
Questions patients ask
'Back lift' is the umbrella; a bra-line lift is the upper back lift whose scar hides under the bra band. Lower back lifts treat the beltline rolls through a different incision.
Flank fullness is treated with liposuction, or with a lower back lift / extended tummy tuck when it's loose skin — not with the bra-line lift, which treats the upper back.
Usually in two stages for safety — lower body lift first, upper body lift 3–6 months later — because the combined operating time exceeds the safe limit for most patients.
