Lower back lift: beltline rolls, lower body lift and the extended tummy tuck

Rolls that sit at the beltline are not a bra-line problem. They hang from the pelvis, wrap the flanks and often continue into the buttocks — and the operation that removes them is the back half of a lower body lift, or the flank extension of a tummy tuck. Here is the honest map.

Why the lower back is a different operation

The upper back's skin is anchored to the ribcage; the lower back's skin is anchored to the pelvis and continues into the buttocks. A beltline roll is pulled by gravity toward the hips, and lifting it means re-suspending the tissue upward from an incision at the beltline — the same line a tummy tuck scar continues into when it is extended around the flanks. That's why the lower back is treated with abdominal contouring, not with a bra-line lift.

Extended tummy tuck versus lower body lift

Extended tummy tuck: the hip-to-hip scar continues around the flanks to remove flank and lower-back rolls; the buttocks are untouched. Right for moderate weight loss and post-pregnancy laxity with flank excess. Lower body lift (belt lipectomy): the scar goes fully around the waist, removing the whole beltline excess and lifting the buttocks and outer thighs. Staging with the upper back

Upper and lower rolls coexist after massive weight loss. The lower body lift is the larger operation and the one that redefines the waist, so it usually goes first; the bra-line lift with arms and breasts follows 3–6 months later as an upper body lift. Doing everything in one session is not safe and is not offered.

Liposuction first? Firm, fatty flanks under elastic skin — the classic 'love handles' — are liposuction cases and need no beltline scar. The pinch test applies to the lower back exactly as to the upper.

Recovery and the trip

A lower body lift is a bigger operation than a bra-line lift: 2 hospital nights, 10–12 nights in Istanbul, bent walking for a week, garment for 6 weeks, exercise at 8 weeks. Is a lower back lift the same as a lower body lift?

A lower back lift is the back half of a lower body lift. On its own it's rarely performed; it is combined with a tummy tuck (extended or circumferential) because the tissue continues around the waist.

Can a bra-line back lift fix lower back rolls?

No — it treats the upper back at bra-band level. Lower back rolls are treated through a beltline incision as part of an extended tummy tuck or lower body lift.

How long is the stay for a lower body lift?
